Lower eyelid bags and abdominal hernias may seem very dissimilar but are actually caused by a similar anatomic problem. Lower eyelid bags are usually the result of fat protruding out from underneath the eye. The eyeball is encased in a cushion of fat which is held back in place by tissue attachments between the tarsus of the lower eyelid and the infraorbital rim bone. With aging, these tissue attachments get weak and this allows fat to extrudes out from underneath the eye through a hole or gaps in them. This is known as herniated eyelid fat which is not so different from bowel which protrudes out through a defect in abdominal wall hernias..
