Patient Testimonials: Chin Reduction

I got a chin reduction with an incision under the chin few months ago. I wanted to give an update. I think Dr Eppley was right about this approach. The chin is smaller. I think it could be a little bit smaller but I definitely think the procedure was worth it. The scar has gotten smaller and I hope it will completely resolve or at least become less noticeable. As always, I’m satisfied and happy with Dr Eppley’s work.  Thanks a lot!

Reducing a large chin has historically been a difficult problem. Traditionally chin reductions were attempted from intraoral approaches, burring down the horizontal portion of the chin bone. While perhaps effective for a few macrogenia (large chin) patients with vertical bone excess (osteotomy with wedge bone removal), this approach is inadequate for most. Horizontal bone burring often left a soft tissue excess that subsequently sagged, creating the classic ‘witch’s chin’ deformity. For many large chins, the tissue problem is multidimensional and involves both bone and soft tissue. This is why a submental approach for most chin reductions offers a more effective solution. From underneath the bone of the chin can be both horizontally and vertically reduced if necessary as well as soft tissue excess removed and tightened. All tissues problems of the large chin can be simultaneously treated. This does result in a submental scar and keeping the length if the scar underneath the arc of the symphysis (curved chin bone) is crucial for a satisfying scar outcome.

While many plastic surgery operations make big changes in how one’s face looks, few operations that are so small can make such a big difference as the corner of the mouth lift. The mouth that turns down, either from how it naturally developed or from aging due to the descent of surrounding tissues, makes one look sad or even mad. This appearance is usually quite contradictory to how one actually feels but the droopy mouth corners create that impression nonetheless. While Botox and injectable fillers can help push up the downturned mouth corners they offer only temporary fixes…and if not done well the mouth lifting effects can be less than desired. A corner of the mouth lift offers a permanent fix to the sagging mouth corner by removing the skin overhang and changing the position of the mouth corner (commissure) upward. Small scars at the corner of the mouth result from the lift but, if well placed and not overdone, they are not a long-term aesthetic issue. When done as a stand alone procedure, corner of the mouth lifts are done as an office procedure under local anesthesia.

While the corner of the mouth lift seems new, it is really an old procedure that was done long before Botox and injectable fillers were even in development. When there was no other way to change the corner of the mouth but skin excision, the old’ valentine’ corner of mouth lift was done. (the skin excision pattern was shaped like a heart)

A very uncommon cosmetic procedure is that of the reduction/removal of prominent blood vessels on the face. By far the most common area is that of the temples and the forehead. Some patients may have prominent veins which are very blue in color. Other patients will have very visible pulsations of the temporal arteries that are aesthetically disturbing on the  side of the head that extends into the forehead. These facial vessels can become extremely prominent when exercising, in hot weather and even when drinking alcohol. While the presence of these blood vessels are normal, they are not usually seen in most people. Why they appear so prominent in some people is not known and is likely just the natural size and blood flow through them or the lack of any subcutaneous fat to camouflage them. While not commonly known or even considered as a cosmetic procedure is the ligation of these vessels to reduce or completely eliminate their prominence due to the blood flow in them. This is usually done as a relatively simple procedure under local anesthesia through very small discrete incisions. The vessels are identified and tied off. In some cases of temporal artery ligation, it must be tied off at multiple points to eliminate backflow which can cause an immediate return of the pulsations. In this more rare case of prominent facial artery pulsations at the side of the mouth, a single point of ligation was sufficient. It is important for the surgeon to be aware of the anatomy of the blood vessels and the location of the facial nerves to avoid any after surgery problems.

Brow Bone Reduction and Forehead Reshaping

‘Had brow bone and forehead bone reshaping done recently by Dr. Eppley. The results are amazing. I had a really big protruding brow bone that made me look like a Neanderthal man. The whole process was very smooth and transparent. A large amount of bone was removed, some of it was reshaped and put back. This way not only sinus size was reduced, but also brow bone on the sides and along orbits was made smoother. Recovery was very quick, I was able to return to work two weeks after the procedure. 

If you have this kind of forehead problem I would recommend this surgery. I would get a 3D CT scan, as Dr. Eppley suggested to me, to make the consultation more productive and results more predictable.

For brow bones that are really prominent, particularly in men, the only really effective reduction method is going to be the osteoplastic bone flap technique. The anterior wall of the frontal sinus (visible brow bone ridge) is very thin, often less than 3 or 4mms, so to expect a significant change from burring is not going to happen. The entire brow bones must be removed, reshaped and then put back in place. By so doing up to 10mms of brow bone setback can be achieved. But no matter how brow bone reduction is done it requires a scalp incision to do it. Whether it is way back in the hairline or along the edge of the frontal hairline (women only) a turn down scalp flap is needed.

While all of this sounds quite ghastly the procedure actually is fairly easy to go though and has a quick recovery. Most patients have little pain after surgery and the biggest issue is some eyelid swelling and occasional bruising. By a week after surgery most people look good enough to walk around in public or even be at work without detectable signs of  having had surgery.

Breast augmentation surgery today has a dirth of implant options ranging from either saline or silicone gel fillers with many different volumes and profiles. But one of the newer breast implant  options is in their shape. Traditionally used as a round implant (doesn’t mean they always look that way when one is standing up), breast implants now come in shaped or anatomic designs. This means that the implant has a tear-drop shape in which essentially 2/3s of the implant volume is in the mid- to lower pole, regardless of whether one is standing up or laying down. Its primary benefit is that it will not leave one with a round looking breast after augmentation, meaning the upper pole will be less full than the lower pole. These shaped implants are good choices for many women over 40 or 45 years old who want a natural look or has a little bit of ptosis or breast sagging. The greater lower pole volume can help pick up some of this loose breast skin and do it without overwhelming the whole breast in size.

All shaped breast implants have a textured and not a smooth implant shell. This is necessary to prevent implant shifting afterwards which can result in a funny looking or cock-eyed breast given the preformed implant’s shape. Textured breast implants must be placed through a lower fold (inframammary) incision to make sure the implant is oriented correctly by direct visual inspection.

My body looks awesome!  My six-pack is showing really well and I have the lines that I always wanted. The lipos got rid of that little lower stomach pooch and those pesky small love handles that I had. The nipple lifts gave my chest a big improvement. I’m getting lots of good comments and compliments again. Back on good diet and in gym. Still under pre-operation weight (was 157 pounds and now 152), but that should come up a little over time. I’m happy that I’m back to having 30”pants loose on me like a couple years ago. Still have just a little tube of tissue on the side of right chest, but interestingly it disappears if I raise back my right shoulder a bit. The entire process took me longer and was harder than I thought it would be…but it is all worth it now!

Body reshaping efforts for men almost always involves liposuction. While for many men these liposuction efforts are primarily about ‘debulking’ and simply trying to get rid of too much fat. But there is a subset of men who are fairly athletic and lean and are having liposuction to create better definition and refinement. This type of liposuction could well be called liposculpture as this is effectively what one is trying to accomplish. These athletic males are trying to be more than just lean but to have a ‘cut’ definition of the chest and the abdominal area. Small cannula liposuction when done in a linear fashion can create the 4-, 6- or 8-pack abs that are very hard to get for some men no matter how much they exercise. Pectoral definition can be achieved by liposuction of the lateral triangle just below the outer border of the pectoralis muscle to create a concavity to better show the muscular border. In some men, particularly those who are a little older, the nipples can drift lower on the chest wall and be a little ‘droopy. By doing a superior crescent nipple lift, they are very effectively moved up a little higher to remove the droop.

My learning experience with where to place it is that no matter where I think I need it, higher is better than lower as the youthfulness part comes from loss higher first then lower so replacing higher instantly youthens.

Second… Doing it under local…So much better!!! I would rather do two or three small timer hits under local than a major hit under general any day… Got to get up, walk out and fly home!

Amy form of surgery is a nerve wracking experience for any patient, but particularly so for a first timer from a distant city who is having a purely elective procedure. The importance of having a staff that is both trained and naturally inclined to be sensitive to these issues and help a patient through the process is the key to a good experience for the patient. While this is no less true than for a patient that lives down the street, the patient from a distant city or country is even more nervous as the very simple things, like sleeping and eating, have to be done in a very unfamiliar environment. Having had many patients come to Indianapolis from all over the world, we have developed a process for international plastic surgery that helps ensure an uncomplicated experience.

Skull reshaping surgery is usually surprisingly easy to go through with very little discomfort afterward. It should not be confused with a neurosurgery procedure where the head is shaved and the skull bones are removed. This is not how it is done at all. It is done on the outside of the skull bones whether it is adding a material or burring down some bone thickness. The bone itself has little to no feeling, it is the overlying soft tissues (periosteum and skin) that has the sensation. The raising of the scalp tissues will usually induce numbness over the elevated skin flaps so even this does not cause much discomfort. Most patients report postoperative discomfort more like a dull headache and many don’t take much pain medicine beyond the first few days after surgery.  If the skull work does not involve the forehead, then there is very little visible swelling as it is back in the hair-bearing scalp. This is why many skull reshaping patients can comfortably return home in just 48 hours after surgery.
